On-Site Safety Audit Prep

A few items the FMCSA auditor needs before the on-site visit. Should take ~15 minutes. Saved automatically when you submit.

One thing to do separately, in your inbox: please open the email titled "ClaimShield: Drug & Alcohol Policy update — please re-sign" and click the link to e-sign the updated policy. That doesn't go through this form.

Carrier Basics

Pulled from the Articles of Incorporation and your tax filing.

From the Articles of Incorporation.
If your most recent MCS-150 number (100,000) is still current, just leave it.

Operations

The most recent trip that crossed a state line.
Within 150 air-mile radius
Beyond 150 air-mile radius
Owner-operators leased to you

Vehicles

Trailer GVWR comes from the metal data plate (usually on the front bulkhead or driver-side rail).

Cab card says 2019, but VIN decode suggests 1989. Which is right? FMCSA portal currently shows 1 power unit / 1 driver. Please file an MCS-150 update at portal.fmcsa.dot.gov reflecting 2 tractors / 3 drivers / current cargo, then upload the confirmation here.

Documents to upload

Photos from your phone are fine. PDFs preferred where possible.

Shows policy effective and expiration dates. The MCS-90 alone doesn't. Both active tractors are owned by Dabas. Auditor will ask for the lease. One PDF per truck/trailer. Combine into one ZIP if easier. Repair logs, oil changes, tire records, etc. for each active vehicle. Call your GEICO claims rep — they have the police report in the loss file. Free, same day.
